is to advise you that I will be closing my practice due to medical disability
effective immediately. It is important
that you continue with appropriate medical care; therefore, you should
establish contact with another physician as soon as possible.

Prema Kothandaraman will be
taking over my practice. You may wish to place yourself under her care, or you

referral. Dr. Kothandaraman’s

you are in a managed care situation, you may need to contact your health care
insurer for a referral.

authorization form to release a copy of your medical records.
